MEDIA RELEASE AND CONSENT

By signing below, I (the "Signer") grant Mario Hernandez, doing business as asecollisiontestprep.com (the "Recipient"), permission to:

1. Photograph, video record, and audio record me during collision repair class activities, demonstrations, and projects.

2. Use my name, image, voice, and likeness in: (a) the asecollisiontestprep.com website, (b) social media accounts associated with the brand (Instagram, YouTube, TikTok, LinkedIn, X/Twitter, Facebook), (c) marketing materials promoting the platform, courses, and services.

3. Edit, crop, alter, or adapt the recordings as needed for publication.

I understand this release is granted without compensation and that my image may appear on a commercial website that sells educational products and earns affiliate revenue.

This release is granted for indefinite duration, but I may revoke it in writing at any time by emailing Mr. Hernandez. Upon revocation, the Recipient will make reasonable efforts to remove previously-published content within 30 days; already-printed materials and content shared by third parties may not be retrievable.

I confirm that I am 18 years of age or older and have the legal authority to sign this release. I have read this release, understand it, and agree to be bound by it. I release the Recipient from any liability arising from the authorized use of the recordings.

This electronic signature is legally binding under the federal ESIGN Act and Florida's Uniform Electronic Transaction Act.
